How Can I Be Insulin Resistant If My A1c Is Normal?

If you've been told your labs are "normal" but you still feel like your body is working against you — stubborn belly fat, energy crashes after meals, sugar cravings you can't outsmart — you're not imagining it, and you're not lazy. You're also not alone. This is one of the most common frustrations I hear from women in their 40s and 50s in my Sherman/Denison practice, and it usually starts the same way: "My A1c came back fine, so my doctor said everything's okay."
Here's the problem. A1c is a lagging indicator. It measures your average blood sugar over the past two to three months — but blood sugar is one of the very last things to go up when insulin resistance is developing. Insulin resistance can be present for 10 to 20 years before fasting glucose ever rises into the prediabetic range, and by the time A1c is abnormal, meaningful metabolic damage has usually already occurred.
Why A1c Misses the Early Stage
Your pancreas is remarkably good at compensating. When your cells start resisting insulin's signal, your pancreas simply produces more insulin to force glucose into your cells and keep your blood sugar looking normal on paper. This compensatory hyperinsulinemia can mask the underlying problem for years, sometimes decades, even while your A1c and fasting glucose both look perfectly fine.
Think of it like a factory running its machines harder and harder to keep output steady, while the machines themselves are slowly breaking down. The output (your blood sugar) looks fine. The strain (your insulin) is the real story — and standard panels almost never check it.
The Labs That Actually Catch It
This is exactly why I order fasting insulin and calculate HOMA-IR on nearly every patient I see for metabolic concerns, not just fasting glucose or A1c.
Fasting insulin — a fasting glucose that looks perfectly normal alongside an elevated fasting insulin can still add up to clear insulin resistance on the HOMA-IR calculation, even though both individual numbers pass as "normal" on a standard lab report.
HOMA-IR — this is a calculated index derived from your fasting insulin and fasting glucose together, rather than a single number viewed in isolation. Generally, lower is better, and a meaningfully elevated score points to insulin resistance well before glucose itself becomes a problem.
Triglyceride/HDL ratio — another window into how your body is handling carbohydrates and fat, often abnormal long before glucose is.
The point isn't to memorize exact cutoffs — reference ranges vary by lab, and your result always needs to be interpreted alongside your symptoms, history, and other labs. The point is that these tests exist, they're inexpensive, and most patients have simply never had them ordered.
Why This Matters in Your 40s and 50s
Perimenopause and menopause change how your body handles glucose. Declining estrogen is associated with reduced insulin sensitivity, shifts in fat distribution toward the midsection, and changes in muscle mass — all of which can nudge insulin resistance forward at exactly the life stage when women are told their labs "look fine." That combination is a big part of why so many of my patients feel like their body changed overnight, even though the labs their previous doctor ran didn't show anything alarming.
What To Do If You Suspect This Is You
Ask for fasting insulin and a HOMA-IR calculation, not just fasting glucose or A1c, at your next visit.
Track your symptoms, not just your labs — energy crashes 1-2 hours after meals, intense carb cravings, difficulty losing weight despite effort, and skin changes like tags or darkened patches at the neck or underarms are all worth mentioning to your physician.
Get a physician who will look past "normal." Insulin resistance caught early is often reversible with targeted lifestyle changes; caught late, it's a much steeper climb.

Can my A1c really be normal with significant insulin resistance? Yes. HOMA-IR and A1c capture different stages of metabolic deterioration and aren't interchangeable — HOMA-IR can flag dysfunction years before A1c or fasting glucose move at all.
What symptoms suggest insulin resistance even with normal labs? Fatigue after meals, difficulty losing weight despite diet and exercise, intense sugar or carb cravings, brain fog, and skin tags or darkened skin folds (acanthosis nigricans) are common clinical clues.
