Why do I have such a hard time losing my weight after 40?

Weight gain is not always about willpower. Insulin resistance, poor sleep, stress hormones, perimenopause, menopause, thyroid issues, low muscle mass, certain medications, nutrient deficiencies, and inflammation can all make weight loss harder.

Many patients are told to “eat less and move more,” but that advice often misses the bigger picture. We look at the signals your body is sending so we can create a plan that supports blood sugar, hormones, muscle, and long-term metabolism.


Perimenopausal/menopausal weight gain, belly fat, and calories alone are not the full story.

Can I have insulin resistance even if my A1c is normal?

Yes. Insulin resistance can begin years before blood sugar or A1c becomes abnormal. In the early stages, your body may keep blood sugar in range by producing more insulin. That higher insulin level can contribute to cravings, weight gain, fatigue after meals, inflammation, high triglycerides, and difficulty losing weight.

That is why we often look at fasting insulin, fasting glucose, A1c, triglycerides, waist measurement, inflammation markers, liver markers, and cardiovascular risk markers together.


A normal glucose and hemoglobin A1c does not always mean normal metabolism.

Do I need medication, a GLP-1, or a lifestyle program?

The answer depends on your labs, symptoms, medical history, goals, and risk level.

Some patients need lifestyle strategy first. Some benefit from medication support. Some need help maintaining weight after stopping a GLP-1. Others need a structured program to rebuild habits around protein, blood sugar, strength training, sleep, and stress.

At Attune, we do not treat weight or blood sugar as isolated numbers. We look at the whole metabolic picture.

Are my symptoms hormones, stress, thyroid, or something else?

The answer is often: more than one thing.

Perimenopause and menopause can affect sleep, mood, weight, insulin sensitivity, muscle, libido, bone health, and cardiovascular risk. But hormone changes are not the only possible cause of symptoms. Thyroid disease, iron deficiency, low B12, vitamin D deficiency, insulin resistance, poor sleep, chronic stress, inflammation, and medication effects can all overlap.

That is why we look at the whole picture instead of blaming everything on age.

Why did my metabolism change in my 40s or 50s?

Midlife hormone changes can affect insulin sensitivity, body composition, sleep, appetite, muscle mass, and where fat is stored. Many women notice more belly fat, more cravings, less muscle tone, and slower recovery even when they have not changed much about their lifestyle.

This does not mean your body is broken. It means your plan may need to change.

A midlife health plan should protect muscle, support insulin sensitivity, improve sleep, reduce cardiovascular risk, and address hormones when appropriate.

Do I need hormone therapy?

Why do I feel older than my age?

Many people are told their basic labs are normal, but they still feel tired, inflamed, achy, foggy, or less resilient than before. Sometimes the issue is not one single diagnosis. It may be a pattern involving insulin resistance, hormone changes, poor sleep, chronic stress, nutrient deficiencies, inflammation, or mitochondrial strain.

Your cells need energy to repair, detoxify, regulate hormones, build muscle, and recover from daily stress. When that system is under pressure, you may feel it long before a standard lab flags a disease.

Can you actually test mitochondrial or cellular health?

There is no single perfect test that tells the whole story of cellular health. Instead, we look at patterns.

Depending on your symptoms and goals, testing may include markers related to blood sugar, insulin resistance, inflammation, nutrient status, oxidative stress, organic acids, glutathione balance, CoQ10, amino acid patterns, lactate/pyruvate ratio, fatty acid oxidation, and biological age testing. 

These tests do not diagnose rare mitochondrial disease. They are used as functional tools to help identify areas where your body may need support.

What can I do to start aging better now?

The basics still matter most: muscle, blood sugar control, sleep, protein, movement, stress recovery, cardiovascular risk reduction, hormone balance when appropriate, and targeted supplementation when indicated.

The difference is that we use your labs, symptoms, and goals to make the plan more personal. Your care plan may include nutrition strategies, strength training, zone-based movement, sleep support, hormone evaluation, cardiometabolic risk reduction, targeted supplements, GLP-1 support when appropriate, and repeat testing to track progress over time.
